The admission process for Montecillo requires the completion of three essentials before an application can be processed.
The Application for Admissions form and the Medical Form must be completed in full and returned to Montecillo. The Medical Form must be completed by the applicant’s own doctor or, if the applicant is in a Public Hospital a letter from the attending Medical Officer will suffice.
The applicant will need a support needs assessment. This is conducted by a Regional Geriatrician and can be arranged by the applicants own doctor, and a copy must be sighted by us prior to admission. If the applicant is in a Public Hospital this can be arranged by the Medical Officer attending.
